Required Client Documentation TO APPLY FOR LIHEAP SPRING SUBSIDY

Application is NOT complete without:

• Proof of residence (photo ID, property tax bill, lease, unopened mail delivered, etc.)

• Income verification (prior month)

• Statement of liquid assets (self-declared)

• Utility bill / lease showing energy responsibility / Utility Verification Form

• Household demographics (household members → Basic Information updated )

• SS Cards for all HH members or eligible immigrant residency documentation

• Component-specific documentation (disconnect notice, eviction, etc.) Not required for

Subsidy programs
